COMMUNITY HEALTH WORKERS

Finding the help you need can be difficult, especially when you don't know where to start, or what services are available to you.

The Fairfield County Health Department is here to help. Our Community Health Worker (CHW) will support you or your family to meet your health, social, and economic needs. The CHW will meet with you to discuss your goals, needs, and limitations, and then connect you with services around the county.


Our CHW services are free to anyone in the county in need of resources.

 

Some services our CHW offers:

  • • Helping you finish health insurance applications and describe insurance to you.
  • • Helping you find food, utility and financial assistance, as well as career and job resources.
  • • Finding you a doctor or other medical services (dentist, eye doctor, behavioral health).
