FOR VITAL STATISTICS RECORDS REQUEST (BIRTH AND DEATH RECORDS)
Pauletta Krieger at 740-652-2836 or you may complete an application for Birth or Death record by accessing the Vital Statistic tab on the FDH homepage. (Vital Records require additional information for processing and payment).
Disclaimer: A person requesting public records need not identify themselves or the reason for which they seek the record. Providing such information to the records clerk is entirely voluntary, although it can often clarify and expedite the release of information. Public records requests can be made in person, over the telephone or online. Records will be released in accordance with the provisions of the Ohio Public Records Act, and may be subject to legal review prior to release.
Public Records copies are provided at $0.11 a page.
Other mediums such as DVD, USB drive, if available may be a different charge.
Prepayment may be required before a request is processed. Payment may be made in person, over the phone or by mail via cash, check, money order, Visa or MasterCard. Checks and money orders should be made payable to the Fairfield County Health Department and delivered to 1550 Sheridan Dr., Suite 100 Lancaster Ohio 43130.
